For New Patients: You will need complete the Patient Health History Form and a Patient Registration Form before you see Dr. Wilkinson or Dr. Snowden. If you wish, you may bring the completed forms with you and avoid having to fill them in at the time of your appointment. Patient Registration Information Form Prior to your appointment, you will need to review our Notice of Privacy Policy . You may read it on our website or obtain a printed copy from our office. You will also need to sign the following three forms: Authorizations And Acknowledgements Form Office Procedure Acknowledgement Authorization to Release Information to Family You may download these forms and bring them with you or complete them in the office. For Established Patients: If any patient information has changed since your last visit (i.e. insurance carrier, address, etc.) please also complete a new Patient Registration Information form. Patient Registration Information Form If you have not already received a copy of our Notice of Privacy Policy , you will need to review this as well. You may read it on our website or obtain a printed copy from our office.
